Northeastern PA Skilled Nursing Home is seeking a Social Service Coordinator

Requirements:

1. Bachelors degree in Social Work or related human service field, including but not limited to sociology, special education, rehabilitation counseling and psychology AND one year supervised social work experience in health care setting working directly with individuals.
2. Experience in managing referrals and complete admission process.
3. Experience in discharge planning
4. Experience with MDS completion
5. Experience in regulatory requirements for chart documentation

Prefer Bachelor's Degree in Social Work and one year experience, if other human service related field, prefer two years experience.
